Include your art website, if you have one.

If you are planning to work with children, you will be required to fill out two additional forms:
1. Pennsylvania Child Abuse History Clearance Form (CY- 113)
2. Pennsylvania State Police Request for Criminal Record Check Form (SP4-164)
These forms will be kept on file for the duration of the year.
